Description – An ACO is a group of providers who work together to improve quality and outcomes, lower cost, and earn financial incentives to meet or exceed performance objectives. Health centers will not be able to rely on traditional revenue models forever. This training will provide an understanding of the APHCA ACO Model, requirements, operational advantages, and proven clinical and financial benefits available to health centers. Engagement with the ACO helps to turn the pressures of CMS and commercial payors into clear and attainable opportunities to deliver better care, strengthen patient relationships, and secure long-term health center sustainability. In partnership with Aledade, APHCA joined a collaborative Accountable Care Organization (ACO) for Medicare patients in 2019. APHCA’s Collaborative ACO will support and expedite health center readiness for Value-Based Care. Participation in the ACO enables health centers to improve care coordination, quality, and health outcomes while strengthening health centers through increased and sustained revenue currently with no financial risk. Leveraging APHCA’s partnership with Aledade, health centers participating in the ACO will be able to identify at-risk patients more quickly, intervene to eliminate gaps in care, reduce unnecessary hospital use, and manage chronic conditions proactively; ultimately being more likely to capture risk adjustment opportunities during the patient’s visit.
