CLIMB: Chronic Care Management Reimbursement Increase
Chronic Care Management (CCM) began in 2015 to improve the quality of life for patients with two or more chronic illnesses and reduce the number of ER visits and hospital admissions and readmissions. Since the program began, CMS has seen favorable results and continued expanding CCM. 2022 is certainly no exception to that rule, with CMS announcing that reimbursement for CCM services for FQHC’s will increase to $76.94 per month per patient, representing an 18% increase over 2021 reimbursement. The CLIMB group will hear from BlueFish Medical, an ongoing APHCA supporter, on the CMS reimbursement updates and understand how CCM can significantly increase health center revenue and improve quality measures. BlueFish Medical reported their FQHC partners to have a higher retention rate in their Medicare population and improved quality of life.
